We typically request the recipient, or intended recipient, to complete an online claims verification form / affidavit. This is an electronic form used to confirm that a shipment was lost in transit or was received damaged. This helps us verify the loss that occurred.
This form is for the consignee to complete, only. The consignee is the person that the shipment was sent to.
What happens if the consignee does not complete the affidavit?
We understand that sometimes the consignee decides that they are not going to participate in the claims process, even those with the best intentions. If they will not verify the claim, please send us your correspondence with the consignee. If a package was lost, how did they alert you to it? If there is a partial loss (shortage) how did you find out about it? Messages between the buyer and seller / shipper and consignee can provide details as to what was shipped and what happened to the shipment.
These messages should be screen shots or screen grabs and include:
- Names of the 2 parties
- Times and dates of the messages
